Euthanasia Information Sheet

General Information:

[1] “Establishment”; “Person”; “Pet”; are used as defined in the “Rules of “GDA” 40-13-13-.01 Animal Protection”

[2] Rules: {A} A “GDA” licensed establishment/person where euthanasia is performed and the layperson performing the euthanasia must comply with “Rules of “GDA” Chapter 40-13-13 Animal Protection” euthanasia section 40-13-13-.08 and the “Georgia Animal Protection Act” O.C.G.A. 4-11-1 euthanasia section 4-11-5.1 and 4-11-5.2;

[3] Documents: {A} “GDA” required information must be recorded on “GDA” approved paper documents or “GDA” approved computer documents; {B} Documents requiring a signature must be signed; {C} All required information documents must be available for inspection at the address listed on the current “GDA” Animal Shelter License;

Regulated Drugs Storage:

[1] The “GDA” licensed establishment/person must have a current D.E.A. registration that contains the following information: {A} Licensed Veterinarian’s name; {B} Physical address of the “GDA” licensed establishment/person where the regulated drugs are stored;

[2] Controlled substance storage must be approved by the “GDA”;

Certification of Layperson Performing Euthanasia:

[1] The “Euthanasia - Certification Letter” must contain the following information: {A} The name of each layperson that is certified to perform euthanasia and the methods they have been certified to use; {B} The “Euthanasia Certification - Letter” must be signed and dated by a licensed Veterinarian; {C} See the “Euthanasia – Certification Letter”;

[2] See the following instructions to add or delete a certified layperson: {A} A new “Euthanasia – Certification Letter” must be generated that list all current certified laypersons; {B} Each new “Euthanasia – Certification letter” supersedes the previous letter; {C} The “Euthanasia – Certification Letter” must be on file at the address listed on the current “GDA” license and a copy faxed or mailed to the “GDA” Companion Animal/Equine office (see the above fax number and address);

Veterinarian or Layperson Performing Euthanasia by Injection: 

[1] Injections: {A} A veterinarian or layperson performing euthanasia must use the order of preference for intravenous, Intraperitoneal, and intracardial injections;

[2] Controlled substance forms: {A} The “Euthanasia – Controlled Substance Form” must be on file at the address listed on the current “GDA” license; {B} See the “GDA” approved “Euthanasia – Controlled Substance Form”;

[3] Signatures: {A} The veterinarian or layperson performing the euthanasia and the witness to the euthanasia are required to sign the “Euthanasia – Controlled Substance Form”; {B} A signature is required – no initials; {C} If a connecting line is used for the witness signature there must be a signature above the connecting line, below the connecting line, and on the connecting line;
